If you don’t have these, please talk to us and we will help you sign-up

Self-Certification

If you are off work sick, you can only be issued with a sick note after you have self-certified for the first 7 days. If you are still unwell after the 7 days, you may then make an appointment to see the GP for a sick note. you can download a self-certification form on the link below and submit it to your employer. Alternatively you can collect one from surgery (charge may apply for photocopying cost).
