New Patient Registration

When you register as a new patient you must complete both registration forms which can be collected at reception or can be printed off by clicking on the links below.  When completed you should return your registration form to Reception and present two forms of identification; one with your name and date of birth and another confirming your address.  These will be checked and handed back to you.  You will then be added to our patient register, and a welcome text message will be sent to you (if you have opted into SMS messages).  This can take up to 48 hours.  You will then be able to book appointments etc.

March  2020 - Please note that during the COVID-19 pandemic you can email these forms to us along with photos of your identification.  This will reduce footfall into the surgery and minimise any risk of transmitting the virus.
