Fridays 9.00am - 12.00pm
The Community Midwife cares for our expectant mothers (from 16 weeks), liaising closely with the doctors. Home delivery / short-stay hospital arrangements can be discussed with the midwife. (Please see information leaflet to describe the roles of members of the team)

New ! Maternity COVID hotline for women that have questions about COVID-19 during the antenatal and postnatal period. It is not for general pregnancy advice.
Women who require a routine antenatal follow up appointment should contact the Acorn Birth Centre on 0161 922 4921.
For pregnancy related concerns that are over 16 weeks gestation should be advised to contact the maternity triage on 0161 922 6175.
When your baby needs their vaccinations you should receive a reminder from the Child Health Department when these vaccinations are due.
Child Surveillance and Post-Natal Check
Congratulations on the birth of your baby! We will contact you via post with dates of your Mum and Baby’s surveillance/post-natal check (at 8 weeks of age). These appointments will be booked together.
Termination of Pregnancy

By appointment with Dr L Palmer on Monday Mornings (Contraception and some skin lesion surgery) or Dr M Laurie on Wednesday Mornings (Joint injections).
Certain minor surgical procedures and joint injections are undertaken at surgery by appointment. The doctor will discuss this if applicable. We can also offer appointments to patients not registered with the practice.
NHS health checks are being offered to people aged between 40 and 74 every five years. The check is to assess your risk of developing heart disease, stroke, kidney disease or diabetes. If there are any warning signs, then together we can do something about it.
The check should take about 20-30 minutes and is based on straightforward questions and measurements such as age, sex, family history, height, weight and blood pressure. There will also be a simple blood test to measure your cholesterol level.
Following the check, you will receive FREE personalised advice about what you can do to stay healthy.

BEAT- Eating Disorders
Beat is the UK’s eating disorder charity. They began life in 1989 as the first national charity for people with eating disorders because of a merger of two local charities. Called the Eating Disorders Association, we have grown and developed over time to become Beat.

If you are not registered at Lambgates Health Centre but want to use our Travel clinic services we are more than happy to cater for your needs but there is some information you need to be aware of.
You need to Pre- register with us then fill in a holiday questionnaire form as this is a private service. A one off payment of £20.00 is required for the nurse Consultation time to go through your forms.
You will the need to contact the surgery 7 days after you have dropped the questionnaire off to arrange payments for any vaccinations prior to your holiday appointment with one of our specialist travel nurses.

There are a number of reports that may be done on behalf of agencies and organisations that are outside of the statutory General Practice contract.
GP's do not have a contractual obligation to fill in these reports and all attract a fee. Fees are charged according to BMA (British Medical Association) guidelines. Forms will be taken all allocated to your regular GP.
Please ensure all contact details are up to date on your medical record so we contact you once they are completed.

You can certify yourself off work for the first 7 days of an illness (including the weekend using a self-certification form (SC1) if self-employed and (SC2) if employed).
You can get an SC1/2 from your work or from your local Revenue and Customs (Tax) Office.
You can download one from www.hmrc.gov.uk/employers/fagsc2.shtml. If your work requires you to have a sick-note during this time a private sick-note will be issued at a cost of £15.00.
If you need to continue to be absent from work after 7 days the Doctor will issue you with a Statement of Fitness for Work Med 3.

The majority of referrals are processed via e-Referral. This means that we put the referral on the system, and then you book your own appointment using the Booking Reference and Password that we provide to you.
We can email the document to you or you can collect from the surgery. Just visit the NHS e-Referrals website to login or if you don't have internet access you can phone the number to book your appointment.
Types of Referrals
Consultant-led appointments (such as Cardiology, Gastroenterology, Orthopaedics, Respiratory)
These are all booked via e-Referrals. Different specialties book appointments in different ways. You may receive the document from us to book the appointment yourself; or it may be booked by the hospital and sent out to you. We will contact you if you need to book the appointment yourself. Hearing Test (Audiology) You can request a hearing test without seeing a GP. Please book an appointment with Nurse Clare or Nurse Hilary who can check your ears prior to referral. Ultrasound Scan Referral will be from your GP. Mediscan come to Lambgates on a Friday afternoon. They also have other clinics around the Tameside & Glossop area on different days. You can book your own appointment via e-Referrals.
Echocardiogram Referral will be from your GP. Northwest Diagnostics are based at a local GP surgery and will send your appointment to you.
Ambulatory ECG Referral will be from your GP. You may be referred for a 24 hour or a 6 day event monitor. Broomwell Healthwatch are based at a local GP surgery and will send your appointment to you.
Community Referrals - Your GP may refer you for other community services such as:

Speech & Language Therapy
Children's Community Nursing Team
